Find answers to questions you may have about Censeo.

No. Completing the Censeo questionnaire is optional but it can provide helpful information to tailor your care. If you choose not to complete it, your journey with SPFT will continue and you will receive the support you need.  
 

It is important that your answers reflect your experience and that you feel that you can be open and honest in your responses. 
However, a trusted family, friend or carer can support you to complete it, and you may want to consider who you could seek support from if you find the questions difficult to answer. Get urgent support if needed.
 

Censeo doesn't ask questions about neurodiversity and will not be used to collect information to support a neurodiversity assessment. However, it could help support the identification of mental health conditions, which people with neurodivergent symptoms or traits could also be experiencing and receive support for. 

Unfortunately, Censeo isn't currently available in other languages. People requiring translation services will be offered an appointment with a practitioner, supported by an interpreter.

The Censeo reports will be added to your Electronic Patient Record (a secure digital platform that holds your personal information, contact details and medical history.)  Read Sussex Partnership's privacy policy to find out more information about how your data is used.

The Censeo assessment survey is provided by our partner Psyomics, who are a data processor for us. The information you provide is not shared with them in an identifiable form. The responses collected from the Censeo questionnaire are stored securely in encrypted servers that are compliant with UK data protection regulations. You can read the Psyomics Privacy Policy on their website.

It is our policy to keep you informed about your care and involve you in the process, however, the report generated by Censeo is aimed at mental health professionals and may contain terminology that is confusing to non-mental health professionals.

Therefore, we feel that it is more meaningful to go through the report with a member of our mental health team. To do this, the information you provide through the questionnaire can be reviewed at your initial assessment. 

If you wish to access the report or any of the information stored about you, you can request information under Data Protection Legislation, by emailing records.management@spft.nhs.uk.
 

The report will be shared with people involved directly in your care, within the service you have been referred too.  If you are referred to one of our partner organisations, such as Time to Talk or Pathfinder, information from the report and your initial assessment will be shared as part of the referral process.
 

Yes, you can turn off notifications / opt out of SMS by using the link on the SMS reminder text messages. After clicking on the link you will then not receive any further messages.
