Community rehabilitation

Our Community Rehabilitation Service is an exciting and innovative service supporting adults with mental health problems in the community.

We know, and the evidence shows, that people who are supported and treated within their own communities, in the least restrictive way, recover better and more quickly. We want to work with you, in your home and community environment, to help you manage your mental health and live an independent and fulfilling life.

We will work with you to develop a care and treatment plan in response to your assessed rehabilitation needs. The service has a long-term pathway where you will be allocated a lead practitioner and consultant psychiatrist, although you may also see other members of the team.

We also have a short-term pathway of up to 16 weeks (up to 12 weeks in Brighton and Hove) where you will continue to be under the care of a lead practitioner and consultant psychiatrist from your referring team, but you will also work with other members of our team depending on your assessed rehabilitation needs. 

West Sussex - We accept referrals from mental health professionals and GPs

East Sussex and Brighton & Hove - We accept referrals from mental health professionals

We work with people within the community which may mean visiting you in your own home or being seen as an outpatient at one of our Trust sites, or meeting in another location such as a café or outside public space.

Our teams are made up of a variety of professionals including doctors, psychologists, nurses, occupational therapists, social workers, peer support workers, support workers, dual diagnosis practitioners, family intervention practitioners, employment specialists, housing specialists, and pharmacists. They will work with you, and together, to provide you with the best and most appropriate care.

We hold referrals meetings twice per week for initial screening of any new referrals, and we aim to respond within seven days for the short-term pathway, and within 14 days for the long-term pathway. Service users will need to already be under the care of secondary mental health services, such as Assessment and Treatment Services (ATS) or Older Peoples Services.
